How to request an itemized bill
Most hospitals send a summary bill by default — a single total without detail. You have the right to request a fully itemized version that lists every individual charge. That line-by-line detail is what makes billing errors visible and disputable.
What is an itemized bill?
- ·Lists each service, procedure, or supply as a separate line item
- ·Contains CPT/HCPCS codes that identify specific medical services
- ·Includes units, modifiers, and billed amounts for each charge
- ·Different from a summary 'Statement of Charges' or Explanation of Benefits (EOB)
Why do you need one?
- ·Many errors, duplicates, and overcharges only appear at the line-item level
- ·Insurance statements and summary bills hide the detail needed for accurate auditing
- ·Providers are legally required to give you an itemized bill upon request — it's your right

Before you upload, check that your document is an itemized bill and not a summary statement.

How to request an itemized bill

Contact your hospital or provider's billing department directly — by phone, patient portal message, or email.

Ask specifically for "a full itemized bill with CPT/HCPCS codes, units, and line-item charges for each service."

Providers are required by law to give you this document upon request. Allow up to 30 days for delivery.
Copy and send to your provider's billing department.
Subject: Request for Itemized Medical Bill Hello, I am requesting a full itemized medical bill for my visit on [DATE]. Please include all CPT/HCPCS codes, units, and billed charges for each line item. Thank you, [YOUR NAME]
Tip: If the billing department says they don't provide itemized bills, ask to speak with a supervisor or request it in writing. Federal law (45 CFR § 164.524) and most state laws guarantee your right to this information.
